Born October 25, 1969 in Beckley, she was the daughter of Larry E. Cook and Ada Maynard Carpenter.
Lesley was a disabled hairdresser.
In addition to her parents she is survived by her sister, Amy L. Cook of White Oak, WV; niece, Ashton S. Kelly of Beckley and uncle, Douglas Maynard of Daniels, WV.
The family is planning a private service.
